Job Title : RN Care Manager – ED / Care Coordination

Duration : 2+ Months

Location : New York, NY 10004

Education : Bachelor's degree required.

Shift Details : M-F 9:00 AM – 5:00 PM

General Description:

Review and evaluate clinical assessments and UAS information for MLTC/MAP members.
Develop, implement, and monitor individualized member care plans.
Coordinate care with PCPs, physicians, nurses, hospitals, and other healthcare providers.
Identify clinical risks and ensure appropriate interventions to prevent unnecessary hospitalizations and ED visits.
Monitor member health status and coordinate preventive and chronic disease management.

Specific Skills to be Added(In case of CB)

Active New York State RN License.
Minimum 5 years of clinical experience as a Registered Nurse.
Hospital experience.
Care coordination / case management experience.
Bilingual in English and Spanish (Spanish-speaking required).
