Responsibilities:
- Provide direct patient care to residents, including assistance with activities of daily living (ADLs), mobility, and incontinence care
- Assist with vital signs, including temperature, pulse, and blood pressure
- Monitor and report any changes in patient condition to the nursing staff
- Collaborate with the interdisciplinary care team to develop and implement individualized care plans
- Provide emotional support and companionship to residents, promoting a positive and dignified environment
- Maintain accurate and timely records of patient care and services provided
- Participate in Shift Reports and other meetings as required
- As a Certified Nursing Assistant at Unity Health and Rehabilitation Center, you will be part of a dynamic and diverse team of healthcare professionals dedicated to providing exceptional patient care
