Job Description
About The Company
UnitedHealthcare At Home works to help people receive support and care in the place they call home. As part of UnitedHealthcare, the organization connects members with services and resources intended to support their health, independence, and well-being. Its approach brings together care coordination, clinical guidance, and connections to community-based services, with consideration for each member’s needs and circumstances. Teams collaborate with members, families, health care providers, and other partners to help make care easier to navigate and more accessible. UnitedHealthcare At Home is committed to respectful, person-centered service and to supporting members as they work toward their health goals.
About The Role
As a Home-Based Care Coordinator, you will help members understand and navigate available health care and support services. You will communicate with members and, when appropriate, their caregivers and care teams to learn about their needs, identify potential barriers, and coordinate next steps. This role requires a compassionate, organized professional who can build rapport, communicate clearly, and manage multiple priorities. You will contribute to a coordinated experience by documenting interactions, following established processes, and connecting members with relevant resources. The position may involve a combination of phone-based coordination, virtual engagement, and home-based activities, depending on program needs and applicable requirements.
Qualifications
Applicants should have a high school diploma or equivalent and relevant experience in health care, human services, customer support, care coordination, or a related field. An associate or bachelor’s degree in social work, public health, health administration, nursing, or a related discipline may be preferred. Experience working with diverse populations, older adults, people with complex health needs, or individuals navigating health and social services is valuable. Candidates should demonstrate strong verbal and written communication, active listening, sound judgment, and a service-oriented approach. The ability to organize work, maintain accurate documentation, use standard computer applications, and follow privacy and confidentiality requirements is essential. Any required license, certification, background screening, travel, or work-location requirements will depend on the specific position and applicable regulations.
Responsibilities
Engage members in respectful conversations to understand their goals, preferences, questions, and support needs. Explain relevant program processes and help members identify appropriate services and resources. Coordinate communication among members, caregivers, providers, and internal teams, with proper authorization and in accordance with privacy requirements. Identify potential barriers to care, such as difficulty accessing appointments, understanding instructions, or connecting with community support, and help determine appropriate follow-up. Maintain timely, complete, and accurate records of outreach, member interactions, referrals, and next steps in approved systems. Monitor assigned tasks and follow up on outstanding requests to support continuity and a positive member experience. Escalate clinical concerns or urgent situations to the appropriate licensed professional or designated team, following established protocols. Participate in team meetings, training, and quality improvement activities, and adapt to updated workflows, tools, and program priorities.
Benefits
UnitedHealthcare offers eligible employees a benefits package that may include medical, dental, and vision coverage; retirement savings plans; paid time off; and resources supporting well-being and professional development. Eligibility, coverage, and plan details vary by position, location, and employment status. Employees may also have access to learning opportunities and supportive programs designed to help them grow their skills and contribute to their communities. Specific benefits will be explained during the recruitment process.
Equal Opportunity
UnitedHealthcare is an equal opportunity employer. Employment decisions are made without regard to legally protected characteristics, consistent with applicable federal, state, and local laws. The organization supports an inclusive workplace and provides reasonable accommodations to qualified individuals with disabilities and to applicants who need assistance during the hiring process. Candidates may request an accommodation through the appropriate recruitment contact.
