  • Support continuity through HTY’s mobile services, Wilmington Comprehensive Community Clinic, and telehealth, as appropriate.
  • Respect patient privacy when communicating with partner organizations and obtain authorization when required.
  • Provide culturally responsive, trauma-informed care to patients from diverse backgrounds.

Documentation and Clinical Operations

  • Complete accurate and timely electronic health records, including assessments, diagnoses, treatment plans, prescriptions, and referrals.
  • Document services sufficiently to support clinical continuity, billing, and program reporting.
  • Follow infection prevention, medication safety, laboratory, and equipment-use procedures.
  • Work with the mobile clinic team to maintain supplies, equipment readiness, and efficient patient flow.
  • Participate in clinical consultation, quality improvement, and required staff training.
