LVN/LPN – Healthcare & Medical Services

Provide compassionate, person-centered nursing care in home and community-based settings. Deliver direct clinical services, administer medications, and monitor individuals’ health status. Support people with complex needs, including behavioral health and brain injury, while maintaining a focus on dignity, empathy, and individualized care.

Key Responsibilities

Provide direct nursing care and assist with day-to-day health needs
Administer medications in accordance with clinical standards
Monitor and document health status, changes, and responses to care
Support individuals with behavioral health and brain injury needs
Collaborate with a multidisciplinary team to promote consistent, quality care

Qualifications

Current LVN/LPN licensure
Strong clinical judgment and ability to monitor health and respond appropriately
Compassionate, person-centered approach to care
Ability to work effectively with individuals who have complex needs
