Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide high-quality patient care in the home setting throughout the Corpus Christi service area. The ideal candidate is independent, dependable, and skilled in home health nursing, including comprehensive patient assessments, care planning, and documentation.

Key Responsibilities

• Provide skilled nursing care to patients in their homes.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Complete admissions, recertifications, resumptions of care, and discharge visits.
• Accurately document patient visits and clinical findings in accordance with agency, state, and federal guidelines.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and families on disease management, medications, treatments, and preventive care.
• Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
• Ensure compliance with all home health regulations and quality standards.
  
  

Required Qualifications

• Active Texas Registered Nurse (RN) License
• Previous Home Health Field RN experience
• Strong clinical assessment and critical thinking skills
• Valid driver's license and reliable transportation
• Ability to work independently while managing a patient caseload
• Proficiency with OASIS documentation
• Experience using Homecare Homebase (HCHB)
• Strong organizational and time-management skills
