About the Role

We are looking for an energetic, compassionate, and clinically experienced Registered Nurse who is passionate about advocating for others and making a meaningful difference in the lives of older adults and their families.

The ideal candidate is an excellent communicator, a strong advocate, and a creative problem-solver who thrives working independently. They enjoy building relationships, coordinating resources, and developing support networks that help clients achieve the best possible outcomes.

As an RN Advocate, you will play an essential role in helping clients and their families navigate healthcare decisions, coordinate services, assess risks, and develop individualized plans of care.

The role is part time, approximately 20-25 hours per week, travel time and mileage paid for.  Compensation is based on experience, Assistance with CEUs

Key Responsibilities

• Conduct comprehensive in-home assessments for new clients.
• Collaborate with clients and their families to develop individualized care management plans.
• Prepare detailed recommendations reports based on each client’s assessment, needs, preferences, and goals.
• Conduct follow-up visits to coordinate, implement, modify, and evaluate care plans while assessing health, cognitive, and safety risks.
• Serve as an advocate and liaison between clients, medical providers, and other members of the healthcare team.
• Accompany and transport clients to medical appointments and other appointments as needed.
• Provide guidance, recommendations, and support to clients’ personal and professional support networks.
• Offer guidance and recommendations to guardians as appropriate.
• Collaborate with fiduciaries, powers of attorney, and other key stakeholders to support the client’s best interests.

Qualifications

• A genuine passion for serving older adults and helping them maintain their dignity, independence, and quality of life.
• An active Registered Nurse (RN) license in the state of Ohio.
• At least 10 years of nursing experience, with a strong clinical background.
• Experience in geriatrics, dementia care, hospice, and/or mental health is strongly preferred.
• Willingness and ability to conduct in-home and facility visits, primarily throughout the north and west Columbus areas.
• Strong communication, critical-thinking, organizational, and problem-solving skills.
• The ability to work independently while collaborating effectively with clients, families, and other professionals.
• A flexible schedule and willingness to respond to occasional evening or weekend needs, recognizing that the needs of our clients may arise outside traditional business hours.
