Role:

Physical Therapist – Digital Health Specialist

Responsibilities:

Deliver outpatient rehabilitation through in-clinic and virtual care modalities
Perform comprehensive patient evaluations and develop individualized treatment plans
Utilize telehealth and remote monitoring tools to monitor and support patient progress
Collaborate with a multidisciplinary team to ensure coordinated care
Accurately document patient care in the Electronic Medical Record (EMR)
Participate in ongoing professional development and education
