Job Responsibilities

• Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
• Education of patients and their families on treatment plans, exercises, and self-management strategies.
• Maintain accurate and timely documentation of patient records, treatment sessions, and progress notes.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Adhere to all regulatory and professional standards in the delivery of services.

Qualifications

• Active and unrestricted Physical Therapy license in the state of practice.
• Doctorate in Physical Therapy (DPT) from an accredited program.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal abilities.
• Experience in home health care or outpatient settings preferred.
• Ability to work independently and as part of a team.
• Current CPR certification.
