Qualifications

• PharmD degree from an accredited pharmacy school.
• Active pharmacist license in the state of employment.
• Minimum of 2 years of clinical pharmacy experience in a healthcare setting.
• Strong knowledge of pharmacotherapy and medication management practices.
• Excellent communication, interpersonal, and analytical skills.
• Experience with electronic health record systems and clinical decision support tools.
• CERTIFIED in medication therapy management (MTM) is preferred.
