Responsibilities

Provide nursing care to residents in a long-term care setting.
Administer medications and treatments according to provider orders and facility protocols.
Monitor residents' conditions and report changes promptly.
Maintain accurate nursing documentation and resident records.
Collaborate with the healthcare team to support resident safety and quality of care.

Requirements

Active LPN license valid for practice in Wisconsin.
Recent long-term care or nursing home experience.
Strong resident assessment, medication administration, and documentation skills.
Ability to work 12-hour day shifts for the duration of the assignment.
