The Home Health Scheduler is responsible for coordinating and assigning patient visits to the appropriate field staff while ensuring schedules are organized, accurate, and meet patient care needs. This role requires strong organizational and time management skills, attention to detail, and the ability to manage changing priorities and schedules throughout the day. The Scheduler also serves as a key point of contact for patients, field staff, and other team members, providing excellent customer service, clear communication, and timely support across all lines of business within the Agency. Success in this position requires someone who can multitask, solve scheduling challenges, and remain flexible and professional in a fast-paced healthcare environment while helping ensure patients receive the care they need when they need it.

What You’ll Do

As a Home Health Scheduler, you will:

• Coordinates referrals and schedules patient visits with appropriate field staff based on Clinical Manager guidance, including contacting patients/caregivers, assigning ordered disciplines in the EMR, optimizing geographic routes, and managing ongoing schedules.
• Follows established home health scheduling processes for all Medicare, Medicaid, private insurance, and private pay referrals.
• Provides excellent customer service by educating customers about Agency services, coordinating across service lines, and communicating effectively with internal and external customers.
• Works collaboratively with the management team to improve customer satisfaction and office processes.
• Responds to patient and client needs by providing accurate information within the scope of the role or referring questions to the Clinical Manager or appropriate resource.
• Documents patient and customer communications, including follow-up actions and outcomes.
• Communicates individual needs to the Clinical Manager and maintains confidentiality at all times.
  
  
  

What You’ll Need

• High school degree required, college degree preferred, or equivalent years of training or work experience.
• Minimum of one year health care/business experience in a fast-paced, customer service environment.
