Keep this page open until you have your form. Your answers stay in this tab only, so nobody using this computer after you can see them: closing the tab or the browser clears them. Going back and forward between steps, or reloading, is fine.

Step 1 of 13

The job you are applying for

Copy these from the advert exactly. Use a separate Z83 for each post.

Exactly as it is printed in the advert.

A date, "Immediately", or the notice you must give your employer, e.g. "One month's notice".

Your name and ID

Write them exactly as on your ID or passport: no nicknames, no short forms. The form says your details must match it.

All of them, as on your ID.

Leave it empty if you are not South African and give your passport number below.

Race, gender and disability

The department needs these to follow the Employment Equity Act, 1998. The form says everything on it is treated with the strictest confidentiality.

Race
Gender
Do you have a disability?

Citizenship

Are you a South African citizen?
Do you have a valid work permit?

Your record

Answer every question. A yes does not end your application: the form says a criminal record is weighed against the job's duties, and these answers count only where they relate to the post. A false answer can disqualify you.

Have you been convicted or found guilty of a criminal offence (including an admission of guilt)?

The box on the form is small: keep it short. If you need more room, write "See attached page" and add a page with the details; the form allows that.
