Job Description
Physical Therapist – Digital Health Specialist delivers evidence-based outpatient physical therapy across virtual and in-clinic settings.

- Evaluate patients and develop, deliver, and progress individualized treatment plans.
- Treat a diverse caseload using digital platforms and remote monitoring tools.
- Use EMR to document care, track outcomes, and adjust the plan of care based on patient response.
- Collaborate with a multidisciplinary rehabilitation team to support safe, coordinated care.
- Educate patients on home programs and strategies to improve return to function and independence.
- Licensed Physical Therapist with strong clinical assessment and treatment planning skills.
- Comfort using digital health tools, remote monitoring workflows, and electronic medical records.
- Ability to provide patient-centered education and support adherence to home programs.
- Excellent communication and teamwork skills in a collaborative care environment.
