About the Opportunity:

The SENIOR WELLNESS COORDINATOR role is dedicated to enhancing patient outcomes following psychiatric hospitalization by facilitating follow-up care appointments and supporting discharge planning. This position plays a key role in helping members transition smoothly from inpatient behavioral health facilities to appropriate outpatient services, directly impacting the organization's HEDIS goals and the well-being of its members.

Responsibilities:

• Conduct outreach to inpatient behavioral health facilities to obtain complete follow-up appointment details
• Support arrangement of appropriate follow-up care post-behavioral health hospitalization
• Assist facilities in identifying in-network outpatient providers when follow-up services are not yet established
• Identify and address barriers to obtaining or maintaining follow-up appointments, including transportation and financial needs
• Provide facilities with information on existing providers and resources to aid discharge planning
• Follow up on incomplete or unreported appointment or discharge information
• Assist in identifying health-related social needs affecting transition to outpatient care
• Respond to facility requests for discharge planning assistance and resource referrals
• Maintain accurate documentation of outreach, appointments, referrals, and outcomes
• Collaborate with internal partners and facilities to ensure continuity of care
• Manage assigned cases and outreach activities per established workflows and quality standards
• Communicate professionally and compassionately with providers, facilities, internal partners, and members
• Perform other duties as assigned

Requirements:

• High School Diploma or GED
• Minimum 2 years of experience in behavioral health
• At least 1 year of computer proficiency and telephonic customer service experience
• Experience handling sensitive member and provider issues confidentially
• Access to a quiet, secure workspace capable of protecting PHI
• Reliable high-speed internet connection approved by the organization
• Ability to work a 40-hour week during designated time zone hours (CST)

Preferred qualifications include managed care experience, knowledge of HEDIS, provider outreach experience, familiarity with community resources, and medical or behavioral health setting experience.

Pay Range and Compensation Package:
