Job Description
Clinical Pharmacist – Inpatient Psychiatry
TRx Integrated Pharmacy Services LLC
Location: Vermont Psychiatric Care Hospital (VPCH) | Berlin, VT
Schedule: Full-Time, Onsite | Monday–Friday, 8:00 AM–4:30 PM
Salary: $135,200–$145,600 annually
Anticipated Start: December 2026
About the Opportunity
TRx Integrated Pharmacy Services LLC is seeking a motivated Clinical Pharmacist to join our pharmacy team at Vermont Psychiatric Care Hospital (VPCH), a 21-bed inpatient psychiatric hospital in Berlin, Vermont.
This is an excellent opportunity for community or retail pharmacists interested in transitioning into hospital pharmacy, as well as experienced hospital pharmacists seeking a more specialized clinical role with a predictable daytime schedule.
VPCH offers a unique practice environment combining meaningful direct patient care, interdisciplinary collaboration, and hospital pharmacy operations. With a small, focused patient population and a supportive pharmacy team, pharmacists have the opportunity to develop specialized clinical skills while maintaining a healthy work-life balance.
Prior hospital or psychiatric pharmacy experience is preferred but not required. We value strong clinical fundamentals, sound professional judgment, effective communication, and a willingness to learn. Training and mentorship will be provided.
What You’ll Do
As a Clinical Pharmacist, you’ll work closely with the Director of Pharmacy, an experienced pharmacy technician, psychiatrists, nursing staff, and other healthcare professionals.
Clinical Pharmacy Services
* Participate in weekly interdisciplinary treatment team meetings, working directly with hospitalized patients and their care teams.
* Review medication regimens for safety, effectiveness, drug interactions, and therapeutic appropriateness.
* Perform admission and discharge medication reconciliation.
* Review laboratory results, therapeutic drug levels, and medication monitoring parameters.
* Provide recommendations regarding medication selection, dosing, titration, and adverse-effect management.
* Develop familiarity with psychiatric medications, including antipsychotics, mood stabilizers, long-acting injectables, and other commonly used treatments.
* Assist with medication management for common medical conditions such as diabetes, hypertension, and hypothyroidism.
* Document clinical assessments, interventions, and recommendations in the electronic health record.
Hospital Pharmacy Operations
* Review and verify medication orders.
* Oversee medication dispensing, distribution, and inventory management.
* Collaborate with and provide oversight to pharmacy technicians.
* Support automated dispensing systems (BD Pyxis ES) and electronic health record workflows (CPSI/TruBridge).
* Participate in medication safety, quality improvement, and regulatory compliance initiatives.
Transitioning from Community Pharmacy?
We recognize that many community pharmacists have valuable clinical experience but limited opportunities to practice in an inpatient setting.
Skills developed in retail pharmacy—including medication counseling, identifying drug interactions, communicating with prescribers, managing complex medication regimens, and ensuring medication safety—provide a strong foundation for this role.
VPCH’s specialized psychiatric focus and relatively small patient population can make the transition more approachable than entering a large, high-acuity general hospital.
You’ll receive orientation and ongoing support from a Director of Pharmacy who also transitioned from community pharmacy, along with assistance from an experienced technician familiar with the hospital’s systems and workflows.
You do not need to be an expert in psychiatric pharmacotherapy, SOAP documentation, or hospital pharmacy software before applying. We’re looking for someone with strong fundamentals, a genuine interest in clinical pharmacy, and the motivation to develop these skills.
Schedule and Work-Life Balance
We prioritize a collaborative working environment, predictable hours, and flexibility whenever patient care and staffing requirements permit.
* Monday–Friday, approximately 8:00 AM–4:30 PM
* No routinely scheduled evening or overnight shifts.
* Small, 21-bed inpatient setting with a focused clinical scope.
* Generous paid time off and separate paid sick leave.
* Direct patient care and meaningful participation in treatment decisions.
* Supportive, close-knit pharmacy team.
Shared After-Hours On-Call
The Clinical Pharmacist shares scheduled after-hours on-call responsibilities with the Director of Pharmacy.
On-call responsibilities primarily involve urgent clinical, medication access, or operational concerns that cannot be resolved through routine processes.
During assigned rotations, pharmacists must remain reachable and available for onsite response if necessary.
Historically, the on-call workload has been very limited, with infrequent calls and rare onsite responses.
Compensation and Benefits
* Competitive salary: $135,200–$145,600 annually, based on qualifications and experience.
* Generous PTO: Up to 208 hours (26 eight-hour days) annually, accrued biweekly.
* Paid sick leave: 40 hours (5 eight-hour days) annually, separate from PTO.
* Paid holidays: Six designated holidays annually.
* Professional development: Opportunities to gain specialized psychiatric pharmacotherapy experience, expand clinical skills, and participate in medication safety initiatives.
* Work-life balance: Predictable weekday schedule with flexible leave scheduling when coverage permits.
Please note: TRx does not currently offer employer-sponsored health insurance. A retirement plan is being considered but has not yet been established.
Qualifications
Required
* PharmD or Bachelor of Science in Pharmacy from an accredited pharmacy program.
* Active, unrestricted Vermont pharmacist license, or eligibility to obtain licensure before beginning pharmacist duties.
* Strong medication knowledge, clinical judgment, and commitment to patient safety.
* Effective communication and interpersonal skills.
* Willingness to learn inpatient clinical and operational workflows.
* Ability to collaborate effectively within a small healthcare team.
* Availability for onsite work and shared after-hours on-call responsibilities.
Preferred, But Not Required
* Hospital, inpatient, or psychiatric pharmacy experience.
* Familiarity with psychiatric medications and therapeutic drug monitoring.
* Clinical documentation or electronic health record experience.
* Residency training or board certification.
Community and retail pharmacists are strongly encouraged to apply. Previous hospital experience is not required, and training will be provided to support a successful transition.
