What it is
The Personal Data Sheet (PDS) is CS Form No. 212 of the Civil Service Commission. The current version on the CSC website is the Revised 2026 form, with a guide to filling it out. (csc.gov.ph)
Who asks for it
Government offices ask for it when you apply. The Philippine Sports Commission, for example, asked applicants for a duly accomplished PDS and a Work Experience Sheet attached to CS Form No. 212. (psc.gov.ph)
Some offices ask for it again after a job offer. The Department of Energy lists the Personal Data Sheet among its pre-employment requirements. (prod-cms.doe.gov.ph)
How to get it
Fill it out on a computer, in the MS Excel file from the CSC website. (csc.gov.ph)
Do not leave any entry blank. Write N/A where something does not apply to you. (csc.gov.ph)
Write full names of schools, positions and offices. Do not abbreviate. (csc.gov.ph)
When you apply for a vacant position, send the Work Experience Sheet (WES) with the PDS. It should list only work experience that fits the position. (csc.gov.ph)
Sign it, or put your right thumb mark if you cannot sign, in the boxes provided, and give a government ID number and its date of issue. (csc.gov.ph)
Attach a passport-sized ID photo or an unfiltered digital picture, 4.5 cm by 3.5 cm, taken within the last six months. (csc.gov.ph)
Print only the extra sheets you actually wrote in. (csc.gov.ph)
Be careful
A false statement in the PDS or the Work Experience Sheet can lead to an administrative or criminal case against you. (csc.gov.ph)
